The male hormone moves through the body as free testosterone (about 2% in normal men) or bound to one of two plasma proteins. Only in its free state can testosterone interact with cellular receptors (e.g., exert a muscle-building effect). Although testosterone levels have dropped significantly, the aromatization of testosterone to estradiol is maintained or even accelerated, and more testosterone is also aromatized in adipose tissue. This leads to an excess of estradiol and, consequently, to an unfavorable testosterone-to-estradiol ratio in the blood.
